Terms & Conditions
- The annual enrollment fee must be paid in full at sign-up and is non-refundable.
- Plan coverage lasts one full year from the first date of use and benefits do not roll over.
- Exams, x-rays, cleanings, and fluoride applications must occur during the enrollment year.
- Patients are responsible for scheduling and keeping their appointments.
- Annual enrollment fees and coverage terms are subject to updates.
- Cannot be combined with dental insurance, other offers, or discounts.
- Discounts apply only to services provided at Legaspi Family Dental and not to referred specialists.
- Treatment related to workers’ compensation is not covered.
- All treatment costs must be paid in full at the time of service.
- Plan membership is non-transferable and remains valid only for active participants.

- You are responsible for booking and keeping your appointments.
- A fee may apply for missed appointments without at least 24-hour notice.

Additional Members: $36/month
- Two six-month check-ups
- Three to four cleanings per year
- Annual x-rays (bitewings, PA’s, panoramic)

- 15% off cash price of advanced treatments (fillings, crowns, extractions, implants)
- 25% off cash price of deep cleanings (SRP/scaling, root planing)
